Ⅰ．はじめに

　小児においては天幕上の腫瘍に比し天幕下の後頭蓋窩腫瘍が多く，小児脳腫瘍の50-77％を占めている^1，19）．しかも小児の後頭蓋窩腫瘍ではその半数をcerebellar astrocytomaとmedulloblastomaが占め^{6，16，18，24）}，extracerebellarのtumorは少なく，その多くが第4脳室に関連した腫瘍または脳橋などの脳幹部のgliomaである．これらの腫瘍の中でpontine gliomaはその局在上，手術以外の補助療法に頼らざるを得ず予後もあまり期待できない．しかし小脳半球のastrocytomaは手術のみによってもある程度の遠隔を期待しうるものである．一方第4脳室に発生する腫瘍となると手術は困難ではあるが，必ずしも不可能な症例ばかりでなく，またradiationなどの補助療法でもある程度の効果を期待できる場合も多いので，その治療法の取捨選択にはなお多くの問題がある．しかしなお満足すべき治療成績は得られてはいない．そこで今回はこれら小児第4脳室腫瘍の自験例の遠隔調査を行い，特に年齢，病悩期間，脳室底浸潤の有無組織像，手術方法，放射線療法などとの関係について検討を加えたので報告する．

A follow-up study was made on 74 cases of fourth ventricle tumor in children under 15 years old, treated in Tohoku University Hospital from 1950 to 1974 with special references to the method of treatment and the factors contributing to prognosis.

1. Seventy four (29%) out of 251 children with intracranial tumor had fourth ventricle tumor. These 74 cases were consisted of 38 cases (51%) of medulloblastoma, 23 cases (31%) of astrocytoma and 13 cases (18%) of ependymoma. The ratio of boys to girls was 1.7 : 1 and a peak of incidence could be seen around the age of 6 to 8 years.
