Ⅰ．はじめに

　頸動脈および椎骨動脈を流入血管にもつ頭蓋外の動静脈奇形（以下AVMと略す）あるいは動静脈瘻（以下AVFと略す）は，頭蓋内のそれと比較し稀なものである．

　ここで言うAVMおよびAVFの定義は次のように理解している．動脈と静脈が直接交通しているものがAVFであり，動脈と静脈の間にmalformed vesselsの介在するもの，すなわち血管写上にていわゆるcapillary portionとして造影される部分を有するarteriovenous lesionがAVMと定義される^17）．しかし文献的にはAVM,AVFをともにAVMとして記載しているものや^5），AVMをふくめてAVFと記載しているものなど^1，10）いろいろあり，まぎらわしい．そこで著者らは，以下に記すAVMをAVFをふくめた広義のAVMとして述べることにする．

Congenital arteriovenous malformation of the extracranial region is rare.

The case presented here is a woman aged 29. She gave a history of swelling of the scalp at the left temporal region and noticed a rhythmic pulsating noise in the left ear of five months duration, especially when she placed that side of her head against a pillow. Physical examination revealed a pulsating compressible tumor mass measuring 10cm×8cm in the left temporal area of the head. There was a bruit synchronous with the pulse and an associated thrill over the mass.
