Ⅰ．はじめに

　Atlanto-axial dislocationは，その解剖学的性質上多彩な神経症状を呈すること，また外傷を起点として神経症状の発現あるいは増悪をきたすことは既に知られている．この疾患の治療は，その成因ならびに症状の発現の仕方から，環惟・軸惟の支持性を確保あるいは補強し，更に，必要に応じて除圧を行ない脊髄への影響を防止することにある．この目的のための手術法としては，前方到達法^{5，6，8，15，17，20）}及び後方到達法^{2，4，10，13，16，18）}が報告されている．しかし，固定方法，除圧方法を含めて，これらの適応に関しては，いまだ，明確な答えは得られていない^8，14）．

　我々は，外傷により著明な神経症状を呈した，両側椎骨動脈閉塞を伴い，Os odontoideumを有したAtlantoaxial dislocationの1例に前方到達手術を施行したのでその経験を報告し，あわせて若干の考察を加え，治療法選択の一助としたい．

The two approaches are well-known in the surgical treatment of atlanto-axial dislocation, the one is the anterior and the other is the posterior approach. The indication of these methods is still controversial.

This report concerns with one case of atlanto-axial dislocation with the separate odontoid and bilateral obstruction of both vertebral arteries, which was successfully treated by transoral decompression and fusion between the bodies of the atlas and axis.
