The operative treatment of atlanto-axial disloca-tions differs greatly whether the dislocation is reducible or non-reducible. Unlike the treatment of reducible dislocation, the treatment of non-reducible dislocation should be focussed to immediate decompression to relieve cord compression, and, secondary, to achieve fixation to prevent recurrent dislocation.

It is the purpose of this communication to describe a case of 52 year-old woman with non-reducible anterior dislocation of atlas on the axis due to the separate odontoid and the secondary osteoarthritic changes of the atlanto-axial joint. The minimum sagittal diameter at atlanto-axial level was 3 mm, where a complete myelographic obstruction was noted. One-stage posterior procedure including the foramen magnum and C 1 decompression and the occipito-C 2 fixation with wiring and acrylic proved to be successful with post-operative 12 months-follow-up.
