整復不能なAtlanto-Axial Dislocation—Atlanto-Axial Dislocation （環軸変位）の研究：その3

NON-REDUCIBLE ATLANTO-AXIAL DISLOCATION

　この論文の表題に，「整復不能な」（non-reducible）と，ことわつた理由には，それだけの意味がある。その意味とは，atlanto-axial dislocationには整復可能なものと（reducible），整復不能なもの，とがあるが，整復が可能か否かによつて，病態生理や手術法が，まるで違つてくるからである。整復が可能なものでは，脊髄に加えられる圧迫は"intermittent"であるが，他方，整復が不可能なものでは，脊髄への圧迫は"constant and unre—lieved"である。また，整復の不可能な場合では，脊髄への圧迫を解除しようとしても，整復可能な場合と違つて，頭蓋直達牽引などによつても整復されないために脊髄は圧迫されたままである。そこで，早急に手術的な減圧術が要求される。

　以上を要約すると，atlanto-axial dislocationの手術法は，整復が可能か，否かによつてことなり，整復の可能なものは，整復して固定術を行うべきである。整復の不可能なものは，脊髄の減圧を第1とし，再発を防ぐ目的で固定術を行うべきである。ところで，後者の場合の手術のアプローチについては，多くの論議がある。現在行われている方法には，transoral, transcervicalに大孔や環椎・軸椎に到達して減圧固定を行うanterior appro—ach^2）〜5），あるいはposterior approachによつて大孔減圧・Cl椎弓切除・後頭骨頸椎固定術^6）〜10），さらには，この両者を併用する方法^4），などを挙げることができる。

The operative treatment of atlanto-axial disloca-tions differs greatly whether the dislocation is reducible or non-reducible. Unlike the treatment of reducible dislocation, the treatment of non-reducible dislocation should be focussed to immediate decompression to relieve cord compression, and, secondary, to achieve fixation to prevent recurrent dislocation.

It is the purpose of this communication to describe a case of 52 year-old woman with non-reducible anterior dislocation of atlas on the axis due to the separate odontoid and the secondary osteoarthritic changes of the atlanto-axial joint. The minimum sagittal diameter at atlanto-axial level was 3 mm, where a complete myelographic obstruction was noted. One-stage posterior procedure including the foramen magnum and C 1 decompression and the occipito-C 2 fixation with wiring and acrylic proved to be successful with post-operative 12 months-follow-up.
