An operated case of 13-year-old girl with occipito-atlanto-axial dislocation was reported, whose com-plaints were the weakness of both legs and the right arm. Roulette tomography of superior cervical spine showed the os odontoideum, the hypoplasia of pos-terior atlas arch and the block vertebra of C2 and C3 clearly.

Posterior limb of foraman magnum was rongeured for the purpose of decompression of spinal cord and posterior fusion between cranium and third vertebra with bone from tihia was performed.

Though the respiratory disturbance and tetraplegia as the postoperative complication were appeared transiently, she is getting well now, four months after operation.
