脳動脈瘤手術後，反対側に硬膜外血腫の発生した1例

A Case of Epidural Hematoma that developed after the Aneurysmal Clipping on the Contralateral Side

Ⅰ．はじめに

　硬膜外血腫は，その大部分は外傷性であり，非外傷性のものとしては頭蓋周囲の炎症に伴うもの^7,8），血液疾患や心血管系の手術などに伴う凝固機能の異常によるもの^4），などが報告されている．開頭術の合併症として，開頭部位より隔った部位に硬膜外血腫の発生することは従来より文献上に報告されている．しかし，それらの例は，術中に脳の退縮を図るためにburr holeより脳室穿刺を行っている場合が大部分で，硬膜外血腫とburr holeとの関係を示唆する例も少なくない^1,2,3,9）．また，硬膜外血腫による症状の発現も，術中に起こる急激かつ不明の脳腫脹として^5），または，術後早期の神経症状の悪化として起こる場合^6）のように，比較的早期にみられている．今回われわれは，左内頸動脈—後交通動脈の動脈瘤に対し低体温麻酔下でクリッピングを施行し，術後約2週間目に，軽度の左片麻痺を伴って発症した術後硬膜外血腫の1例を経験したので若干の文献的考察を加え報告する．

A very rare case of subacute epidural hematoma which developed two weeks after clipping of cerebral aneurysm of the contralateral side was described.

A 55-year-old housewife was admitted on December 15,1976, suffering from severe headache with neck stiffness that suddenly appeared several hours prior to admission. Nausea and vomitings were present, but no motor and sensory disturbance were noted by the patient herself. Lumbar puncture performed on admission yielded bloody CSF with opening pressure of 250mmH2O. Neurological examination disclosed nothing abnormal apart from mild nuchal rigidity.
