Ⅰ．はじめに

　脳動脈瘤は通常その発生原因より，先天性，動脈硬化性，細菌性，外傷性などにわけられる．これらのうち頭部外傷後に発生する外傷性脳動脈瘤はまれなものとされている．外傷性脳動脈瘤に関する現在までの報告では，その大部分がcavernous portion，頭蓋外などの内頸動脈領域，および中硬膜動脈，浅側頭動脈，顔面動脈などの外頸動脈領域に発生したものであり，前，中あるいは後大脳動脈などの領域に生ずる末梢性脳動脈瘤の報告は非常にめずらしい．

　開放性頭部外傷における本疾患の出現頻度についてはベトナム戦争での報告があるが，穿通性脳損傷2,187例中2例にこのような末梢性脳動脈瘤が発見されているにすぎない^9）．

A case of traumatic cerebral aneurysm associated with growing skull fracture was reported.

This patient was 1-year-old girl, who fell down from the stairs and struck her head on the floor on June 3, 1972. After this accident her general condition was almost good, but a slight left hemiparesis was in exis-tence since then. A right carotid angiography was per-formed on June 17, 1972, and a pea-sized aneurysm arising from the right callosomarginal artery was revealed. And also, growing skull fracture at the frontal region was observed in plain skull X-rays on June 3, and June 29.
