椎骨血管腫による脊髄障害—症例と文献的考察

Spinal Cord Lesion due to Vertebral Hemangioma:a Case Report

Ⅰ．はじめに

　脊椎の血管腫は剖検では10％前後^9，23）の高頻度にみられるとされるが，脊髄障害などの臨床症状を呈する症例は外国文献にも少なく，本邦においては，整形外科領域から散発的に症例報告がみられるにすぎない^{11，14，16，1，22）}．われわれは対麻痺をきたした65歳の主婦に対し，手術により血管腫と診断し，第8胸椎の部の減圧手術を行なって効果をあげた症例を経験した．単純X線像上特徴的な所見があり，あらかじめこの種の疾患に精通していれば，術前診断も困難ではないと思われた．また，術後の血管造影にて興味ある所見を得たので加えて報告する．

A 64-year-old female suffered from symptoms of com-pression myelopathy at D 8th vertebra received neurolo-gical study, including selective spinal angiography. Neurological symptoms were indifferent to that of extra-dural compression myelopathy of any cause, however, as a pathognostic finding in the thoracic spine X rays spindle shaped shadow in the paravertebral soft tissue surrounding compressed D 8th vertebral body was found, which was quite resemble to the X ray findings of tuber-culous spondylitis or metastatic tumor of the spine.
