慢性硬膜下血腫に合併した脳表仮性動脈瘤—外傷性中大脳動脈瘤に関する一考察

Pseudoaneurysm of the Cortical Artery associated with Chronlc Subdural Hematoma: A consideration about traumatic middle cerebral artery aneurysm

I．はじめに

　慢性硬膜下血腫と中大脳動脈皮質枝の仮性動脈瘤の合併例は稀であり，わが国において木矢（1978）^28），園部（1981）^51）の報告があり主として外傷性と考えられている．われわれは最近特異なCT像を示した慢性硬膜下血腫と中大脳動脈皮質枝の仮性動脈瘤の合併例を経験したので，これを報告し，外傷性中大脳動脈瘤を文献的に考察し，さらに慢性硬膜下血腫と仮性動脈瘤との関連についても考察したので報告する．

A 62-year-old man was admitted to our hospital, since chronic subdural hematoma was showed after he complained of mild headache and nausea. Two years ago he had head trauma at the left parietal region after drinking. On admission there was no paresis and mild choked disc is detected. Computed tomography with contrast enhancement showed abnormal enhancement and left chronic subdural hematoma. Cerebral angiography showed an aneu-rysm of the angular artery. Operation was clone and the aneurysm was trapped. Aneurysmal wall was histologically a pseudoaneurysm. Post-operative result was good.
