はじめに

　Myelin-associated glycoprotein（MAG）は分子量約100,000 Da（100 kDa）で，約30％の糖を含有しているミエリン特異蛋白のひとつである^1）。ミエリン中の含有量は，中枢神経系でもミエリン総蛋白の1％弱と微量であり，末梢神経系ではさらに微量である^2）。しかしミエリン内にて神経軸索周辺に集積しているという分布様式などから，ミエリンと神経軸索との相互関係に重要な物質ではないかと推測されていた^1）。最近MAGとneural cell adhesion moleculesとの交叉抗原性の存在が報告され^3），細胞間認識におけるMAGの役割がさらに重要視されつつある。また多発性硬化症（MS）^4,5）や進行性多巣性白質脳症（PML）^6）などの脱髄疾患において，脱髄巣周辺のMAGが他のミエリン蛋白よりも広範囲に減少していることが報告されており，脱髄過程のあるステップにて何らかの役割を担っているのではないかと考えられている。最近，IgM型M蛋白血症に多発性神経炎型の末梢神経障害を伴った患者にて，その血中M蛋白そのものがMAG抗体であったという症例が報告され^7），ヒトの疾患で出現したモノクローナル抗MAG抗体が，末梢神経障害に関与している可能性が示唆され注目されている。最近われわれは各種の中枢および末槽神経系の脱髄疾患において。患者の血中に抗MAG抗体が出現していることを報告し^8），抗MAG抗体と脱髄疾患との関連を示した。

　MAGに関する最近の重要なトピックスは，ミエリン^9〜11）もしくはミエリン以外の神経組織^{3，12〜14）}，および免疫組織歴^15〜17）などの神経系以外の組織との交叉抗原性に関する報告である。ミエリン内では末梢神経ミエリン中の強酸性糖脂質^9，11）と複数の低分子糖蛋白^10），神経系内では先に述べたneural cell adhesion molecules3）など，神経組織以外ではヒト末梢血中のnatural killer細胞^15〜17）や悪性腫瘍組織^18）など，かなり多彩な交叉抗原性を示している。これらの組織内の共通抗原決定基を含む労子は，これまで報告されているものでは，糖脂質の糠鎖や，分子量の異なった糖蛋白など，MAGとは異なったものである^{3，10，11，18〜20）}。

　The myelin-associated glycoprotein (MAG) is a quantitatively minor constituent of myelin with an apparent molecular weight of 100,000 Da. Since immunocytochemical studies demonstrated its presence in the periaxonal regions of myelin sheaths in human tissue, it may be involved in glia-axon interactions. Recently the cross antigenicity between MAG and neural cell adhesion molecules has been reported. Therefore, roles of MAG in cell to cell interactions in nervous system are speculated to be important. The immunocytochemical studies have shown that the MAG is decreased in periplaque regions of some multiple sclerosis lesions that appear normal when immunostained for myelin basic protein (BP). These results suggest that loss of immunoreactivity of MAG in the plaque periphery may be early biochemical change associated with demyelination. Interest in the role of MAG in human demyelinating diseases also drives from recent case reports on patients with chronic polyneuropathy and monoclonal anti-MAG antibody as IgM paraprotein.

　Another recent topic on MAG is its cross antigenicity with various tissues such as the immune system especially natural killer cells, neoplasmas such as lung cance tissues and cell adhesion molecules.
