排尿の生理

Physiology of micturition

はじめに

　膀胱内に徐々に貯留した尿により膀胱内圧が10mmHg前後に達すると，尿意を催し，排尿反射によって尿道を通して外部に尿が排出される。排尿筋自身の自発性収縮はきわめて弱く，排尿を引き起こすほどの収縮力は発現しない。排尿の発現には主として骨盤神経を求心路，遠心路とする反射性の排尿筋の大収縮とそれに伴なう内尿道口の開口，外尿道括約筋の弛緩による尿道抵抗の減少が必要である。

　排尿反射の中枢は古くより腰仙髄に存在するといわれてきたが，より上位の脳幹にある反射中枢が関与し，また，大脳皮質によって随意的にある程度排尿機構が調節されている。ここでは主として排尿の神経性調節機構の生理について現況を概説する。

Abstract

The current knowledge of the neural mechanisms that control the micturition reflex may be sum-marized as follows:

1.As the urine collects in the bladder,the pelvic vesical sensory impulses excite the pudendal motoneurons innervating the external urethral sphincter muscles.This reflex prevents the urine from escaping the bladder by increasing the ure-thral resistance.

The closing of the internal urethral orifice may be partly due to enhanced reflex outflow of the hypogastric nerves produced by the pelvic and hypogastric centripetal impulses from the bladder.
