典型的なOPCA病変を伴なった線条体—黒質変性症の1剖検例—とくに，被殻に出現している顆粒の性状について

An autopsy case of striato-nigral degeneration combined with typical olivo-ponto-cerebellar atrophy, with special reference to so-called putaminal pigments

I．はじめに

　線条体黒質変性症（striatonigral degeneration，以下SND）とオリーブ・橋・小脳萎縮症（olivopontocerebellar atrophy，以下OPCA）の病変が同一例においてしばしば合併して出現し，また，Shy-Drager症候群が剖検上しばしばSNDやOPCAの病理組織像を伴なっていることから，近年これらの疾患を包括する概念としてのmultiple system atrophyという名称が用いられる趨勢にある。いっぽう，これら3者は必ずしも合併するとは限らず，また合併していてもそれぞれの病変の強さに強弱があり，臨床像の上からもかなりの差異があるのが通例である。最近われわれは生前にSNDと診断され，剖検上明らかなSNDの組織像に加えて典型的なOPCA病変を示し，かつ脊髄病変を認める1例を経験した。さらに近年SNDの剖検例で被殻に出現し，注目されているいわゆるputaminal pigmentsについても組織化学的および電子顕微鏡的検索を行なった。その結果，本例の線条体とりわけ被殻においてneuromelaninと思われる顆粒や鉄陽性顆粒の出現を観察した。以上の所見に基づき臨床病理学的検討と文献的考察を行なった。

　A 54-year-old man developed gait disturbance and he was diagnosed as Parkinson's disease. In the early stage of the disease, L-DOPA was somewhat effective. When L-DOPA was not given for two weeks by accident, he fell into bedridden due to pronounced rigidity and akinesia, which were recovered by readministration of this drug. Bradykinesia and rigidity were most prominent symptoms. Tremor was hardly noticed. In a relative short time, his difficulties progressed and pronounced.

　One and half years later, he became enable to walk without support.
