4．振戦および類似の現象についての小脳系（Cerebellar Circuit）の役割について—リズム形成の障害を中心として

1）私は，パーキンソニズムにおける定位視床手術の経験と，1-Dopa治療の経過における病像の分析から，振戦と強剛の神経生理学的機序について明らかにしえたところを，これまですでに繰り返し記述した。本稿においてはそのような所見や，解析の過程を繰り返し述べることは避けたいが，その要点を述べると次の二つに纒めることができる。すなわち，ⅰ）中枢神経内において，少なくとも視床thaIamusのレベルにおいて，筋緊張系（tonus系）と振戦系（tremor系）とは異なった局在を持ち，それは視床腹外側核（nucl. ventralis lateralis，VL核）と外側中間核（nucl. ventralis intermedius，Vim核）であり，前者は主として蒼球（pallidum，GP）よりの投射を受け，後者は小脳よりの投射を受けると考えられる。VL核とVim核とはよく知られるように前後に接して位置しているが，後者，Vim核，に対する定位手術は，パーキンソン振戦，小脳性振戦（企図振戦，姿勢振戦など），Benedikt症候群の振戦，Wilson病の羽搏き振戦などのいずれに対しても著効を示す。ⅱ）このような二つの系，すなわち振戦系の機能的異常の程度と，末梢における筋緊張レベルの程度の相関によって，種々の振戦の現象型phenotypeが決められると考えられるのであって，パーキンソニズムにおける振戦はその一つの発現型にすぎないと考えられる。

From experiences of human stereotaxis on parkinsonism and other tremorous movements with use of microrecording and -stimulation, the thalamic structure related to tremor, Vim (Ventralis Intermedius Nucleus), can be differentiated from VL (Ventralis Lateralis Nucleus). The latter mainly concerns to rigidity and receives the afferent inflow from the pallidum. Various clinical tremor can be aleviated by the same surgical target on Vim, and the analysis was made that the tremor producing mechanism will be related to some dysfunction of the cerebellar hemispheric circuit.
