I．はじめに

　ジストニー（Dystonie，dystonia）という言葉の概念には，現在混乱がある。その混乱はdystonia musculorum deformans（Oppenheim，1911）の示す病像が多彩であることと，歴史的に，この疾患と症候名としてのジストニーの内容と一致しないことがもたらした点が大きい。

　神経学の症候とその機序の理解には，ジストニーにみられるような，歴史的な理解を必要とするものが少なくない。本稿ではこの概念について，歴史的にみた変遷と，病態生理に基づいたジストニーの理解を，dystonia musculorum deformansと症候性ジストニアの相違，局所性ジストニーのとらえ方に焦点を当てて述べる。

The term dystonia is defined as a type of motor disoder characterized by slow, writhning, involuntary movement of extremities, neck or trunk (dystonic movement) and fixed abnormal posture (dystonic posture) due to abnormal involuntary contraction of skeletal muscles resulting from diseases of the basal ganglia. On the otherhand, the disease “ dystonia musculorum deformans” (Oppenheim, 1911) or idiopathic torsion dystonia is characterized by variable motor disorders different from symptomatic dystonia caused by lesions involving the putamen.
