はじめに
アテトーゼとジストニーはともに不随意的な筋収縮に基づく症候であり,大脳基底核を主体とする病変によって発症するが,さまざまな類似点と相違点を有する。症候学的にはアテトーゼが緩徐な不随意運動であるのに対して,ジストニーは不随意運動というよりも,不随意姿勢または異常姿勢ともいうべき持続性の筋緊張異常である。しかし,時間的な要素を除くと,両者がよく類似した症候学的な形態を呈することも多い。両者は電気生理学的にも多くの共通点を有する。本稿ではアテトーゼとジストニーの中枢神経系での病態機構の概略についてまず述べ,ついで両者の電気生理学的特徴と最近における知見の進歩について述べたい。
1 Electrophysiological characteristics of athetosis
1) Athetosis is characterized by incessant slow involuntary movements predominantly in distal parts of the body in association with spontaneous non-reciprocal electromyographic (EMG) grouping dis-charges of the duration around 1-3 sec synchronously in agonist and antagonist muscles. 2) The stretch reflex in athetotic muscles in resting state varied from hypoactive to hyperactive. Only when associated with rigidity or spasticity, the tonic or phasic stretch reflex was enhanced, respectively.
