在宅慢性神経疾患患者のコミュニケーション障害への対策

How to cope with communication problems in home care of patients with neurological diseases.

I．はじめに

　慢性神経疾患の在宅ケアで問題になることの一つに，コミュニケーションの障害がある。慢性神経疾患患者のケアにさいして，たとえば患者の種々の要求や訴えを聞くことは介護のうえでもきわめて重要であり，また逆に患者にいろいろな情報や指示を与えることも大切である。コミュニケーション障害とは，このような出力面と入力面の両方向の問題を含み，具体的にいえば，ことばの生成の障害と聞きとりの障害が主体となる。

　このうち最も多いのは，神経疾患に伴う発声発語器官の運動障害による話しことばの生成障害であり，一般に麻痺性構音障害とよばれる^1）。このような患者では，さらに四肢の運動障害を伴っていわゆる閉じ込め状態（locked-in state）にあることも少なくない。いずれにしても麻痺性構音障害の場合，患者の精神活動は正常レベルに保たれるのが普通であるために，ことばを用いての意志や感情の伝達が不能であることによる患者の精神的苦痛や不安，不満には著しいものがある。また脳血管障害の例では，単なる運動障害に加えて失語の問題も起こりうる。こうなると，コミュニケーションの能力はさらに限定される。

In the treatment of patients with chronic neurological diseases, home care is one of the most important factors for their rehabilitation. In order to achieve effective home care for these patients, it is indispensable to maintain their daily communication ability. In patients with dysarthric problems of neurological origin, it is often the case that oral communication is restricted due to neuromotor deficits in the articulatory organs. Further, since the incidence of dysarthric problems is relatively high in aged subjects, hearing impairment due to presbyacusis may also affect their communication ability.
