　症例は49歳，男性。尿閉を主訴に当科を受診した。直腸指診にて，直腸前壁に手拳大，弾性硬，表面不整な腫瘤を触知した。血清PSA値に異常を認めず，経直腸的前立腺超音波断層法にて腫瘤の推定重量は311g，内部は不均一であった。経直腸的針生検では確定診断に至らず，病理解剖にて前立腺平滑筋肉腫であることがわかった。前立腺平滑筋肉腫の本邦報告例は現在までに60例あり，自験例が61例目である。

A 49-year-old man with urinary retention was referred to our hospital in November 1999. A histopathological study of the transrectal needle biopsy specimens revealed a sarcoma with spindle-shaped cells. Since dis-tant metastases (lung, liver and bone) were pointed out,the patient received CYVADIC combination chemother-apy consisting of cyclophosphamid, vincristine, adriamy-cin and DTIC. He died 4 days after the initiation of che-motherapy due to sepsis. The tumor had directly invaded the wall of the urinary bladder and the rectum.In addition, there were distant metastases in lung, liver and bone from tumor of the prostate.
