　コックパウチ造設6年8か月後に輸入脚カラーが脱出し，これを中心に結石を形成した症例を報告する。摘出困難のため，内視鏡的に砕石をくりかえしたが，砕石も困難となり，脱出1年11か月後に開腹にて，カラーの摘出を行った。輪状の輸入脚のカラーは，ほとんどパウチ内に脱出しており，中央の穴に少量の粘膜が覆っているだけであった。カラーを切断することにより容易に摘出することができた。輸入脚を損傷することもなかった。

We report a case in which a stone was formed on the erodedcollar of afferent nipple of Kock ileal reservoir 6 years and8 months after the surgery. Due to the difficulty of theremoval of the eroded collar, endoscopic lithotripsy wasrepeatedly performed, but finally failed. Then open surgicalremoval of the collar was performed one year and 11 monthsafter the erosion. At operation, circular collar was almostcompletely eroded into the pouch and only a small amountof mucous menbrane covered the central hole of the collar.By cutting the collar, the stone was easily removed. Theafferent nipple valve was not injured.
