症例1は42歳男性。20代後半から右陰囊腫脹を自覚していたが，1年前から増大傾向を認め2013年2月当科初診となった。症例2は56歳男性。1年前から右陰囊腫脹を自覚しており，急激に増大したため，2013年2月当科初診となった。両症例とも悪性疾患を否定できず，高位精巣摘除を施行した。病理結果はいずれも黄色肉芽腫性精巣上体炎であった。

Two patients, a 42-year-old man and a 56 year old man, presented complaining right intrascrotal mass with no other symptoms. They visited our hospital because the mass enlarged gradually. They had no symptoms of UTI and inflammatory reaction was negative. Right high orchitectomy was performed because testicular tumor could not be denied on ultrasonography and computed tomography. Histopathologically, the diagnosis was xanthogranulomatous epididymitis. Xanthogranulomatous epididymitis is extremely rare. To our knowledge, only 8 cases have been reported.
