55歳，男性。検診エコーにて右副腎腫瘍を指摘され受診。CTにて肝と癒着した5.5cm大の副腎腫瘍あり。肺にも多数の小結節を認めた。副腎腫瘍の鑑別を行い，MRIや血清抗体検査などから副腎エキノコックス症と診断し，手術を施行した。世界において，副腎のエキノコックス症の報告はこれまで2例のみである。肝からの浸潤を伴った右副腎腫瘍の場合，居住地や画像診断などから，エキノコックス感染症を鑑別に挙げる必要がある。

Multilocular echinococcosis is exclusively located in liver by intaking the eggs of echinococcosis from the mouth. We experienced a rare case of adrenal gland infection with echinococcosis. A 55 year-old man came to our department because of abnormal echoic study. The abdominal CT showed that the right adrenal gland was swollen, irregular, with many cysts and calcification. A part of adrenal gland was adhered to liver. The chest CT showed many metastastic lesions in both lungs. We resected the right adrenal gland and a part of liver and the diagnosis of multilocular echinococcosis was confirmed.
