症例は54歳，男性。2005年より血液透析を施行していた。2009年10月，左腎盂腫瘍に対して左腎尿管全摘除術を施行した。病理結果は尿路上皮癌（pT4pN2）であった。同年12月のCTで多発性肺腫瘍を認め左腎盂癌の多発性肺転移と診断し，2010年1月よりgemcitabine（GEM）/cisplatin（CDDP）：GC療法を開始した。肺転移は部分奏効を得た。有害事象は骨髄抑制を認めたが12か月間安全に施行できた。

A 54-year-old man under hemodialysis for chronic renal failure underwent left nephroureterectomy for left renal pelvic cancer. The pathological finding was urothelical carcinoma, G2（pT4pN1）. Three months later, CT revealed multiple lung tumors. We began a chemotherapy regimen of combined gemcitabine and cisplatin（G/C）under the diagnosis of urothelial carcinoma with lung metastasis. Twelve months later, almost all lung tumors disappeare, and pertial response（PR）was obtained, al though grade 3 neutropenia and grede 3 thrombocytopenia occurred. We conclude that G/C chemotherapy can be used safely and effectively for under hemodialisis patients.
