塩酸ピラルビシンによるアナフィラキシーショック

Anaphylactic shock induced by pirarubicin hydrochloride

　われわれは，塩酸ピラルビシン投与によるアナフィラキシーショックの2例を経験したので報告する。症例1はTURを施行した83歳，男性。術後23日目に塩酸ピラルビシン静脈内投与を開始した。2回目の投与の際にアナフィラキシーショックが出現した。症例2はTURを施行した79歳，男性。術後9日目に塩酸ピラルビシンの膀胱内投与を開始した。投与14回目にアナフィラキシーショックが出現し，その際のカテーテル留置時に塩酸ピラルビシンを含む残尿を認めた。塩酸ピラルビシン投与によるアナフィラキシーショックは，静脈内投与では本邦2例目，膀胱内投与例は1例目である。

We report two patients with anaphylactic shock induced by pirarubicin hydrochloride. The first patient was 83-year-old man who had been treated with transurethral resection（TUR）for bladder tumor. Twenty-three days later,intravenous injection of pirarubicin hydrochloride was performed. At the second injection,anaphylactic shock occurred. The second patient was 79-year-old man who had been treated with TUR-BT. Bladder instillation therapy using pirarubicin hydrochloride was performed nine days after intravesical operation. Anaphylactic shock occurred after 14th intravesical injection,with apparent residual urine containing pirarubicin hydrochloride. This is the second reported case of anaphylactic shock induced by intravenous injection of the pirarubicin hydrochloride,and the first reported case of intravesical injection,to our knowledge.
