トキソプラズマ症

TOXOPLASMOSIS

　トキソプラズマ症はToxoplasma gondii （以下，Tpと略す）によつてひき起こされるzoono-sis （人獣共通感染症）である。Tpは感染スペクトルがきわめて広く，自然感染はヒトを含みほとんどの哺乳動物や，多数の鳥類ならびに一部の冷血動物などにも発見されている。しかし，本症がごく最近にいたるまで注目されなかつたのは，病原体であるTpの検出が比較的困難であつたことと，ヒトにおける症状が割合に特異性が少ないため，他の疾患と誤認されることが多かつたためであろう。しかし，最近診断に有力な種々の免疫学的検査法が考案され，Tp症の疫学および臨床についての飛躍的な研究がおしすすめられてきた。今回，一症例に特有の眼底変化をともない，Senear-Usher症候群様，急性湿疹様およびジベル・バラ色枇糠疹様の多彩な皮疹をつぎつぎに示した本症を経験したので報告する。

The disease in a 28-year-old housewife was reported. Besides severe systemic and ocular sym-toms, and generalized lymphadenopathy, she showed the skin lesions which looke dlike Senear-Usher's Syndrome, acute eczema, and pityriasis rosea Gibert subsequently. Histologic specimen showed the eczematous or psoriasis-like epidermodermatitis, but it had also some characteristics -an inflammatory reaction deep in the dermis, primary degenertaive change of the epidermis, and foreign body or tuberculoid granulomatous reaction.

Although the hemoagglutination titer of the serum was abnoamally high and went well with the clinical course, toxoplasma gondi was failed to be proved from the lesions by the animal inoculation test. Pyrimethamine and sulfamonomethoxine showed a marked therapeutic effect.
