Porphyria cutanea tarda symptomatica—自験例とその成因とくにporphyrin代謝について

PORPHYRIA CUTANEA TARDA SYMPTOMATICA

まえがき

　尿中にポルフィリン体またはその前駆物質の排泄増加をきたす疾患のうち，1次的または遺伝的にポルフイリン代謝異常をきたす群をポルフィリン症と呼び，他のある種原疾患により2次的に惹起されたポルフィリンの代謝異常により尿中排泄増加をみるものをポルフィリン尿症と呼んでいるが，後者はポルフィリン体による皮膚症状ならびに精神神経症状などは示さず，ポルフィリン体自身その原疾患の副産物と考えられる。いずれにせよ，尿屎中のポルフィリン体の消長は，その代謝率の大きい骨髄赤血球系と肝の代謝異常を反映するもので，他の組織におけるポルフィリン代謝にはほとんど関係しない。

　ポルフィリン体は生命の基本的機能である呼吸と密接な関係にあるヘモグロビン，ミオグロビン，チトクローム属，カタラーゼなどと絡がるので，あらゆる組織または種々の疾患においてその代謝異常をきたすことが予想されるが，現在のところまだその排泄遊離体を通して間接的にその異常を知るのみである。しかしながら近年生化学の大きな成果の1つとして，ポルフィリンの生合成過程が明らかになり，組織呼吸との関係から代謝疾患としてのポルフィリン症の解明は一段と重要性を増すとともに，従来の古典的な臨床的分類より脱皮して生化学的基礎的知見を基として，ポルフィリン代謝経路の障碍の場およびその閾値により，疾患の動態ならびにその成因を究明する時期に至つたと考えられる。

A 38-year-old man of porphyria cutanea tarda with the pseudoscleroderma-like lesion on the face and neck was reported. The clinical findings,-a group of yellowish white papules on the upper eyelids and neck, the ichthyotic lesions on the chest and back, a yellowish discoloration of the vocal cord, and the roentgenographic shadow in gyrus hippocarnpi,-resembled those of the lipoid proteinosis.

Laboratory tests revealed 6.2 mg/day, 2.98 mg/day, 1.41 mg/day, and 0 mg/day of uroporphyrin, coproprophyrin, ALA and PBG in urine respectively. CP (++) and PP (trace) in the faeces were proved.

The diagnosis of porphyria cutanea tarda symptomatica was given in consideration of the patient's chronic liver function disturbance and the personal history (a drunkard without fami-lial history of porphyria).
