　はじめに

　複数の上皮小体に腫大がある場合には，一般的に過形成を考えるが，稀にいずれもが腺腫の場合がある。このような症例は，術前の診断は困難であり，病理組織診断が重要となる。

　今回われわれは，初回手術で正常部位の上皮小体腫瘍を摘出した後にも上皮小体機能亢進症が改善せず，^99mTc-MIBIシンチグラフィーで精査したところ異所性上皮小体腫瘍を認め，これを2回目の手術で摘出した後に血清カルシウム値が低下した上皮小体の重複腺腫の1例を経験した。その確定診断に至るまでの経緯を呈示し，若干の文献的考察を加えて報告する。

Double parathyroid adenomas are rare cause of pri-mary hyperparathyroidism. In May 1999, a 59-year-old woman visited our hospital. Hypercalcemia and elevated serum parathyroid hormone level indicated a diagnosisof primary hyperparathyroidism. Ultrasonography and CT scan showed an enlarged left upper parathyroid gland. Although she had undergone the first operation in August 1999, she had persistent hypercalcemia for two years. In April 2001, the technetium 99m sestamibi scan showed an ectopic parathyroid tumor in the mediasti-num. In August 2001, she underwent the second opera-tion and the tumor was removed, resulting in normocal-cemia. Both of those histological examinations revealed parathyroid adenoma.
