前頸部皮下血腫を初発症状とした上皮小体腺腫の1例

A Case of Parathyroid Adenoma Presenting as a Spontaneous Subcutaneous Hematoma in the Anterior Neck

I．はじめに

　上皮小体の囊胞性疾患は稀な疾患であり，高カルシウム血症や高PTH血症を伴わないことが多く，頸部エコー検査時に偶発的に発見されたり手術後の病理検査で初めて診断されることがある。

　今回，われわれは前頸部から前胸部皮下に及ぶ血腫を契機に診断された囊胞形成を伴う上皮小体腺腫を経験したので，若干の文献的考察を含めて報告する。

　A 58-year-old female with parathyroid adenoma presented with the first symptom of spontaneous subcutaneous hematoma in the anterior neck. In the larynx,submucosal hematoma was detected by endoscopy. Cervical CT scanning indicated a cystic mass in the left upper lobe region of the thyroid gland. Hemorrhagic fluid was aspirated from the cystic mass. We suspected the cystic mass as a thyroid tumor,and it was considered that ruputure of the cystic mass caused an extensive hematoma.

　The cystic mass was resected with the thyroid left lobe under general anesthesia. Histopathological examination revealed parathyroid adenoma with cyst formation.
