鼻腔に発生した多形腺腫および悪性多形腺腫の2症例

Two Cases of Pleomorphic Adenoma of the Nasal Cavity

　はじめに

　多形腺腫は耳下腺，顎下腺などの大唾液腺に好発するが，小唾液腺の存在する口腔，口蓋にも発生し，稀に鼻腔，咽喉頭，気管，涙腺にも発生するといわれている^1〜6）。大唾液腺と小唾液腺に多形腺腫が発生する比率は，Bergmanら^7）によると6：1とされている。

　われわれは，鼻中隔に発生し鼻側外切開にて摘出後，再発をみない1症例と，稀な鼻腔原発悪性多形腺腫の1症例を経験したので，若干の文献的考察を加え報告する。後の1例は寺邑ら^8）の報告した症例と同一症例であるが，前報告が途中経過のものであったため，詳細を報告する。

Case 1. A 35-year-old male who complained of nasal obstruction had a tumor arising from the leftside of the nasal septum. The tumor, 20×20×20mm in size, was surgically removed by lateral rhinotomy. There has been no recurrence for 8 years after dissection.

Case 2. A 61-year-old male complained of nasal obstruction due to a tumor with bone destruction. He underwent lateral rhinotomy and radiotherapy, but lymph node metastasis and local recurrence were observed and the patient died.
