　はじめに

　近年，ナビゲーションシステムが内視鏡下副鼻腔手術に応用されるようになってきた^1〜6）。これまでに蝶形洞手術やオリエンテーションの困難な再手術症例などでの使用経験が報告されている^4〜6）。

　今回われわれは，ナビゲーションシステムを用いて摘出した鼻・副鼻腔神経鞘腫の1症例を経験した。本症例では，ナビゲーションシステムを併用した内視鏡下鼻内手術が有用であった。その診断，治療，経過などを呈示し，若干の文献的考察を加えて報告する。

A 51-year-old woman presented with a 4~5-year history of nasal obstruction. Computed tomography showed a mass occupying the right nasal and paranasalcavity In spite of biopsy obtained from the surface of the mass, no definitive histopathological diagnosis was made. The maxillary sinus was opened and the postope-rative histopathological study revealed the mass was schwannoma. Therefore, the second operation was per-formed by endoscopic endonasal surgery under image guidance, and most of the remaining tumor was resected using a powered instrument. The postoperative course was satisfactory. The patient had no recurrence in 6 months following the surgery.
