サイバーナイフが著効した鼻・副鼻腔悪性黒色腫の1例

Cyberknife Treatment for Malignant Melanoma of Nasal Cavity and Sinuses

I．はじめに

　悪性黒色腫は皮膚および粘膜，眼球などのメラノサイトを起源とする悪性腫瘍であり，全悪性腫瘍の中でも非常に悪性度が高く，予後不良な疾患の1つである^1）。頭頸部領域では口腔（口蓋，歯肉），鼻・副鼻腔に発生することが多い^1）。治療は手術を中心とし，化学療法，免疫療法，ホルモン療法，放射線療法を併用する集学的治療が行われている^1）。しかし，頭頸部領域の中で鼻・副鼻腔領域に発生したものについては，解剖学的に眼球，視神経，脳など重要な臓器が近接していることから，十分な安全域をつけての拡大手術は困難になることが多い^1）。

　今回，われわれは，87歳男性の鼻・副鼻腔に発生した悪性黒色腫に対し，サイバーナイフによる治療を施行し，腫瘍の縮小効果および腫瘍出血に対する止血効果を認めたので文献的考察を含めて報告する。

　Malingnant melanoma usually occurs in tissue which contains much melanocytes such as skin,mucosa and eye is one of the most highly malignant tumor and there is no satisfactory therapy ever reported.

　We reported the effectiveness of Cyberknife treatment,which is a new method of robotic radiosurgery,for malignant melanoma.

　An 87-year-old man with malignant melanoma of the nasal cavity and sinuses was treated by Cyberknife.

　This treatment was effective to reduce the tumor size and to prevent the massive bleeding from the tumor.
