　はじめに

　甲状腺の結節性病変に対する超音波検査，穿刺吸引細胞診が一般化したことで，質的診断がより確実になり，良性疾患に対して手術が選択される機会は減少している。良性腫瘍はその腫瘍径が4〜5cmを超えない限り，一般に手術適応とはならず，またバセドウ病も薬剤によるコントロールを慎重に行うことで，手術に至る症例を減少させることが可能である。同じ甲状腺の自己免疫疾患である慢性甲状腺炎は，原則として手術を回避すべき疾患と考えられる。しかし，慢性甲状腺炎の中でも，腫大が徐々に進行し，ときにはその腫大が気管を圧迫して呼吸困難を呈したり，咽頭，食道を圧迫して嚥下障害を引き起こすような場合には手術を選択せざるを得ない。

　最近われわれは，気道，食道への圧迫症状を呈して手術を選択した2例を経験した。この2例を紹介し，その手術術式と手術時期について検討を行ったので報告する。

Two cases of chronic thyroiditis that caused dysphagia and dyspnea arizing compression to the trachea and esophagus. The first patient was a 58-year-old female and the other was a 48-year-oldfemale. The first patient complained of dysphagia and the latter complained of dyspnea. Both patients received total thyroidectomy, and the symptoms disappeared after the operations. Postoperative hypoparathyroidism and recurrent nerve paresis were not observed in both patients. Total thyroidectomy is better procedure than subtotal thyroidectomy and partial thyroidectomy for the patients who complained the progressive thracheo-esophageal symptoms against the thyroid hormone therapy.
