　はじめに

　近年，QOLへの関心が高まる中，嚥下障害に対するリハビリテーションの重要性が注目されている^1〜6）。しかし，嚥下のリハビリテーションの効果判定については，その多くが経験的に行われている傾向にある。より有効なリハビリテーションを実施するためには，客観的な効果判定の指針が確立されていることが望ましい。

　今回われわれは，Arnold-Chiari I型奇形による嚥下障害の1例を経験した。この症例の嚥下障害に対してリハビリテーションを施行し，良好な結果を得た。また，この症例のリハビリテーション前後の嚥下動態の解析結果から，嚥下のリハビリテーションの効果判定の指針について若干の知見を得たので，併せて報告する。

A 36-year-old female patient presented with pro-gressive dysphasia accompanied by type I Arnold-Chiari malformation. The otolaryngological and neurological examinations revealed that the sen-sation and motion of the right pharyngeal wallwere declined, the left sensory dessociation was observed and bilateral tendon reflex was accelerat-ed. The findings of videofluorography revealed that the swallowing reflex was delayed, the degree of laryngeal elevation was reduced and Barium was pooled in the right piriform sinus. Then, the rehabili-tation for swallowing using Mendelsohn maneuver and head rotation to the affected side was done for six weeks. After that, the dysphasia remarkably reduced with the improvement of the swallowing parameters.
