声帯麻痺側の甲状腺腫瘍の存在のため診断が遅れた食道癌傍気管リンパ節転移症例

Delayed Diagnosis of Esophageal Carcinoma because of Thyroid Tumor

　はじめに

　声帯運動障害に伴う嗄声の原因疾患として，耳鼻咽喉科領域では喉頭癌，下咽頭癌，甲状腺癌などの腫瘍性疾患を挙げることができる。しかし，反回神経はその走行が長いため，様々な原因によって麻痺を生じることは周知の事実であり，その背景には頭頸部領域以外にも種々の原因疾患が存在することを忘れてはならない。

　今回われわれは，嗄声を主訴に受診し甲状腺癌の気管傍リンパ節転移による反回神経麻痺を疑わせた胸部上部食道癌症例を経験したので，若干の文献的考察を加えて報告する。

A 58-year-old male was complaining of hoarse-ness. Endoscopic examination revealed fixation of the right vocal cord in paramedian position.

CT, ultrasonography of the neck showed asolid mass lesion in the right lobe of the thyroid gland and lymph node swelling in the right paratra-cheal region. He was initially diagnosed as thyroid tumor and lymph node metastasis following right recurrent nerve paralysis. Right thyroid lobectomy and right regional neck dissection were performed. The histological examinations revealed the thyroid tumor as follicular adenoma and the lymph node tumor as squamous cell carcinoma. The final diag-nosis was thoracic esophageal carcinoma.
