　小脳扁桃の奇形であるArnold-Chiari氏奇形の成因並びに治療に関しては未だに論議の余地があり，今後尚，検討されるべきものと思う。

　本奇形に関する報告は欧米では数多く見られるにも拘らず，本邦に於ては，清水^26），星野^13），深井^9），須田^28），石郷岡^15），徳岡^31）の各1例，田中^30）の2例，計8例を散見するにすぎない。又，本奇形の一成因である牽引説を裏づける脊椎破裂を伴つた症例は本邦では徳岡の1例を除いて他に報告をみない。私共は最近，脊椎破裂を伴つたAr-nold-Chiari氏奇形を経験したので，いささか考察を加え報告する。

The second case of Arnold-Chiari malfor-mation associated with spina bifida in Japan was presented. The patient is 5 years old female with abnormal enlargement of head, lumbodorsal tumor, headache, nausea and vomitting. It is thought that Lichtenstein's "traction theory" is the cause of this case.

Another case aged 6 monthes male occur-ring rapid abnormal enlargement of head af-ter the operation of spina bifida was repor-ted. This suggests that artificial traction of spinal cord on the operation is one of the causes of Arnold-Chiari malformation.
