著しい呼吸困難と嚥下障害を伴った—Arnold-Chiari奇形

Severe Respiratory and Swallowing Disturbance Associated with Arnold-Chiari Malformation

Ⅰ．緒言

　脊髄髄膜瘤を伴ったArnold-Chiari奇形は日常しばしば経験するところであり，近年，脊髄髄膜瘤そのものに対する手術適応，手術時期など種々の観点から議論され，今日一応の規準が出来上っているといえよう．一方，Arnold-Chiari奇形に基づく水頭症に対しては無造作にシャント手術が行われ，それなりの成果はあげているものの，この奇形に伴うことがある脳幹障害に関しては比較的等閑視されているようである．そこで，筆者らのArnold-Chiari奇形の症例のうち，著しい呼吸障害と嚥下障害を伴った代表例2例をあげ，本奇形における呼吸障害と嚥下障害，およびそれらの治療法に関して論ずることにする．

Infantile hydrocephalus caused by Arnold-Chiari malformation are treated by shunt operation nowadays in most institute. As a rare complication, lower cranial nerve palsy and brain stem compression associated with ArnoldChiari malformation have been known. From our experience, we like to present two such complicated cases, and discuss about sign, symptom and management.

The first case was 8 day-old infant who was born with spina bifida and myeloschisis at lumbosacral region. She was transfered because of contaminated wound around myeloschisis and meningitis. Conservative treatment was started.
