Subacute Necrotizing Lymphadenitis

　はじめに

　頸部リンパ節腫脹は様々な原疾患により生じ，悪性疾患や重篤な疾患の1症状である場合もあるので，迅速かつ的確な診断が要求される。その中でも，亜急性壊死性リンパ節炎は比較的経過が長く，診断や鑑別に苦慮することが多い。そこで，最近われわれの経験した亜急性壊死性リンパ節炎症例について，その病態や症状，検査所見などを比較検討し考察したので報告する。

Four cases of subacute necrotizing lymphadenitis were analysed, and the summary of the results was as follows.

In case of multiple node swelling with tenderness, subacute necrotizing lymphadenitis is highlysuspected if the patient does not respond well to antibiotics, and if leucocytosis and higher increase in CRP are not observed and neoplastic findings are not obtained by FNA. Leucocytopenia and a slight increase in monocyte fragments and CRP are most likely diagnostic findings, though they were not very much typical in these 4 patients. Symptoms disappeared spontaneously in 3 months without any treatment.

However, sufficient progress observation is neces-sary, because there are some cases developed severe complications.
