基礎疾患のない健康成人に発症した頸部リンパ節結核の3症例

Cervical Tuberculous Lymphadenopathy in Three Healthy Adults

　はじめに

　国際的な努力にもかかわらず，結核は今なお重要な感染症の1つで，細胞性免疫を低下させる後天性免疫不全症候群（AIDS）の出現に伴い再度世界的に増加する傾向にある^1）。日本では1996年に全国で42,472人の結核患者が新規登録されたが^2），1997年では38年ぶりに結核感染が増加し，42,715人が新規登録されたという報告が先頃なされた^3）。

　東京中心部にある虎の門病院で，続けて健康成人3症例の頸部リンパ節結核を経験したので，今後再び増加する可能性を考え，耳鼻咽喉科における結核の診断，治療，問題点を考察した。

The number of newly registered tuberculosis patients increased in 1997 from the previous year, marking the first rise in the last 38 years. In thesame year three cases of cervical tuberculosis lymphadenopathy in healthy adults were treated in Toranomon hospital. A case which had taken times to be diagnosed showed large multiple lymphade-nopathy that collapsed to the skin and took long time to recover. It was important to make diagnosis and start treatment as soon as possible. It could be necessary to remove lymph nodes if other tests for tuberculosis were negative.
