　はじめに

　抗生物質の進歩とともに耳鼻咽喉科領域の頭蓋内合併症は減少し，特に鼻性頭蓋内合併症は極めて稀となっている。治療の基本は鼻，副鼻腔に対する手術と強力な化学療法と考えられ，必要により脳神経外科的手術を併施することもある。外傷性や手術後に生じた以外の，原因が不明確な頭蓋内合併症（自然発生例）では，診断が遅れがちで重篤な結果を引き起こすことが多いとされている。われわれは副鼻腔炎が原因と考えられた細菌性髄膜炎例を経験したので，文献的考察を加えて報告する。

A 65-year-old male presented with bacterial meningitis secondary to sinusitis in left frontal, bilateral ethmoidal and maxillary sinuses. Therewere no severe infections that could be a focus of bacterial meningitis in other organs including the lower respiratory system. Although conservative therapy was effective at first, meningitis was con-sidered to recur. The patient was operated on for the infected sinuses. After surgery, meningitis has not recurred by the time of this report. From find-ings of the operations, it was thought that the focus of meningitis of this patient was the right maxillary sinus or the left frontal sinus.
