術後急速に全身転移をきたした耳下腺粘表皮癌の1症例

A Case of Mucoepidermoid Carcinoma in the Parotid Gland Showing Rapid Metastases

　はじめに

　大唾液腺腫瘍の部位別発生頻度では耳下腺腫瘍が最も多く，良性腫瘍では多形腺腫，悪性腫瘍では粘表皮癌，腺癌が多い。

　今回われわれは，病理組織学的には低悪性度と分類される粘表皮癌を摘出し，約1か月で全身に転移を認めた症例を経験したので，文献的考察を加えて報告する。

A 88-year-old male with mucoepidermoid car-cinoma in the parotid gland developed facial palsy and the tumor grew gradually. Parotidectomy had not been done. Because he was old and needle biopsy had not showed malignancy. Total parotidectomy was done and 3 months after the surgery pericardial and pleural effusion were observed and the patient died. Autopsy shewed general metastasis in the pericardium, lung, liver, bone and others.
