　はじめに

　鼻副鼻腔における真菌症は日常臨床において時に遭遇するが，その予後は一般に良好である。しかし，稀に骨破壊を伴って周囲組織に浸潤し，治療に抵抗して致死的となる例がある^1）。われわれはこれまでに，後部副鼻腔より眼窩尖端や頭蓋底に浸潤したアスペルギルス症を3例経験した。そのうち，早期に眼窩内容の摘出を含む病変の徹底した郭清を行った2例は救命し得たが，1例は中大脳動脈領域の広範な梗塞をきたして死亡した。今回，これらの症例の治療経過を報告するとともに，本症の診断と治療について考察を加えた。

Three cases of sino-orbital aspergillosis were reported. All of them initially complained of tempo-ral headache and/or ophthalgia, and followed by visual loss and ophthalmoplegia. CT and MRI show-ed a soft tissue invasion into the orbital apex and skull base from the posterior ethmoid and sphenoid sinuses. Aspergillus was identified by either of his-topathological or bacteriological examination. Sur-gery was undergone including an exenteration of orbital contents in all cases, and antifungal chemo-therapy was added. Postoperative course was un-eventful in two case, but the other died of wide-spread infarction of the brain.

Prompt surgical eradication of the lesion is a choice of the treatment.
