眼窩尖端症候群を呈した副鼻腔真菌症の1例

A Case of Paranasal Sinus Mycosis Presenting Orbital Apex Syndrome

I．はじめに

　副鼻腔真菌症は，日常診療においてたびたび遭遇する疾患であるが，ときに眼合併症や致死率の高い頭蓋内合併症などの重篤な合併症を伴うことがあり，的確な診断を行い，それに基づいて適切な対応を行うことが必要である。副鼻腔アスペルギルス症の眼窩内症状の発現頻度は4.3％で^1），眼窩に浸潤したアスペルギルス症の眼窩尖端症候群をきたす頻度は2％と低く^2），本邦における過去10年間で眼窩尖端症候群をきたしたという報告例は5例^3～7）に過ぎず，比較的稀であるが，そのうち2例は頭蓋内浸潤が原因で死亡しており注意を要する疾患である。

　今回われわれは，眼窩尖端症候群をきたした副鼻腔アスペルギルス症の1例を経験したので，臨床経過と若干の文献的考察を加え報告する。

　A 68-year-old female presented with disorders of the optic,oculomotor,trochlear,trigeminal,and abducent nerves,ie orbital apex syndrome. CT scan revealed inflammatory lesions in the maxillary and ethmoidal sinuses extending to the pterygopalatine fossa and into the orbit. After intranasal sinus surgery,The lesion was diagnosed histologically as Aspergillus mycosis. Antifungal agents were therefore administered. At present,neurological deficits were improved,and CT scan showed that the inflammatory findings were improved.
