眼窩尖端症候群をきたした数例についての解析

Clinical evaluation of orbital apex syndrome

緒　言

　眼窩尖端症候群は1896年Rochon-Duvigneand^1）によつて報告されて以来多数の報告がある。特徴的な所見としては視力障害，眼瞼下垂，眼筋麻痺，三叉神経第1枝，第2枝の支配領域の知覚異常などがみられる。その成因には，（1）眼窩内深部の炎性病変，（2）隣接副鼻腔の病変の波及，（3）視神経管，上眼窩裂に近接した頭蓋内病変の波及，（4）腫瘍の転移など種々のものが掲げられているが，おのおの症例についてその原因を明確にすることは必ずしも容易でない。

　今回，著者らは，1973年4月以降本教室において眼窩尖端症候群の16例を経験したが，そのうらの原因の異なる4症例について述べ，16例の原因別分類および鑑別点について比較検討をおこなつたので報告する。

　We observed sixteen cases of the orbital apex syndrome due to paranasal tumor, low-grade nonspecific inflammation of cavernous sinus (Tolosa-Hunt syndrome), sequel of brain tumor and aneurysm of IC-PC. Four cases with different etiological origins are described in detail.

　The first case was caused by the paranasal sinus tumor ascertained by X-ray examinations and rhinoscopy. The second case was the Tolosa-Hunt syndrome in which a number of examinations failed to show positive results, but systemic steroid therapy was very effective.
