　はじめに

　口腔腫瘍や中咽頭腫瘍に対する切除後の再建法として，欠損部が大きい場合には大胸筋皮弁や広背筋皮弁，さらに近年では前腕皮弁，腹直筋皮弁などの遊離皮弁が用いられる。これに対して中規模の欠損部の再建法としては確立した方法がなく術式の選択に迷うところである。広頸筋皮弁は口腔内の再建術式として1978年Futrellら^1）によって提唱され，以後，口腔咽頭における中規模の欠損部の再建法として有用であることが報告されている。今回われわれは舌癌，中咽頭癌，歯肉癌の3症例に対し本法を施行し良好な結果を得たので報告する。

Three patients with head and neck carcinomas underwent platysma myocutaneous flap reconstruc-tion for intraoral and pharyngeal defects after ablative surgery. The primary sites of malignancy were the tongue, the mesopharynx and the lower alveolus. The flaps successfully survived in all the cases. Neither wound dehiscence nor skin necrosis occured in donor and recipient sites. Functional or cosmetic impairment was not observed in donor sites. These results confirmed the reliability and usefulness of this operative procedure. We conclud-ed that the platysma myocutaneous flap is an appli-cable alternative with minimal surgical trauma for the coverage of medium sized intraoral and pharyn-geal defects.
