Reconstruction of the Head and Neck Region with a Latissimus Dorsi Musculocutaneous Flap

はじめに

　広背筋皮弁は長い血管柄を有し，広い筋体と皮膚を採取できる。解剖学的変異も少なく採取手技も容易なため，再建手術においてその適応範囲は広い。頭頸部領域においては，とりわけ頭蓋部の再建材料としてその需要が高い。本稿では主に頭蓋部の再建術における遊離広背筋皮弁の適応について述べる。

　The latissimus dorsi musculocutaneous flap has a long vascular pedicle and a wide muscle body. There is little anatomical variation in the latissimus dorsi and the harvesting procedure is easy, and there is a wide range of applications for a latissimus dorsi musculocutaneous flap in reconstructive surgery. A safe and effective flap is in high demand for reconstructions of the head and neck region, especially in cranial reconstruction. The procedure using such a flap is relatively easy, but it is important to thoroughly simulate and design the required muscle body, the size of the skin island, and the length of the vascular pedicle in advance of the reconstruction. Plastic surgeons who are performing head and neck reconstructions should be proficient in the use of a latissimus dorsi musculocutaneous flap.
