慢性鼻副鼻腔炎患者の頭痛・顔面痛に関する臨床的検討—（その2）非頭痛群との比較

A Clinical Study of Headache and Facial Pain in Chronic Rhinitis and Sinusitis II:Comparison between Pain and Painless Group

　はじめに

　慢性の鼻副鼻腔炎患者の訴える頭痛や顔面痛は程度も責任部位も曖昧なことが多く，その因果関係も不明瞭である。しかし，鼻疾患患者にみられる頭痛の頻度は21.2〜59.4％と意外に高い^1〜3）。それゆえ，慢性の頭痛と副鼻腔炎の関係について明確にすることは，日常の実地臨床において極めて重要なことと思われる。前回，われわれは慢性の鼻性頭痛の臨床的特徴を明確にする研究の一環として，慢性の鼻炎および副鼻腔炎患者の訴える頭痛や顔面痛の臨床的特徴について報告した^4）が，今回は慢性鼻副鼻腔炎患者に前鼻鏡検査，副鼻腔X線撮影，鼻腔通気度測定を行い，頭痛を有する群と有さない群で比較検討し若干の知見を得たので報告する。

In 150 patients with chronic rhinitis and chronic sinusitis, we investigated clinical characteristics between headache group and painless group. Ninety patients, (36 males and 54 females, mean age of 49 years) had chronic headache or facial pain, and 60 patients (24 males and 37 females, mean age of 52 years) did not. No significant differences of age, sex and total nasal resistances were found between the two groups. Extents of X-ray shadow of the right ethmoid and the both maxillary sinuses in the pain-less group were significantly increased. In the pain-less group, rhinoscopic findings showing nasal polyps and hypersecretion, were significant. On the contrary, complaints concerning nasal problems such as nasal stuffiness and post nasal discharge, in the headache group were significant. Mental factors in chronic rhinitis and sinusitis patients accompany-ing headache seemed one of the important causes.
