　はじめに

　急性副鼻腔炎患者における頭痛・顔面痛では，その責任部位や症状の程度との因果関係は比較的明瞭である^1）。しかし，慢性の場合はそれほど単純ではない。欧米では，これら慢性の鼻性頭痛について既に多くの研究が行われてきているが^2〜4），本邦でのこの領域の研究は極めて少ない。われわれは，以前より慢性鼻副鼻腔炎患者の頭痛・顔面痛に関する臨床的研究を行ってきた^5〜7）。欧米では，これら慢性の鼻性頭痛に対して鼻副鼻腔手術が積極的に行われており，その臨床効果も認められている^2）。今回，われわれは慢性鼻副鼻腔炎患者の訴える頭痛・顔面痛が鼻副鼻腔手術によりどのような経過をたどるか検討したので報告する。また，頭痛の出現に心因的な背景やアレルギー性炎症の関与が疑われているので，今回の研究ではその点に関しても検討を行った。

In 75 patients with chronic rhinosinusitis treated with surgical procedures, changes of craniofacialpain before and after operation were studied by means of questionnaire. Twenty-seven patients of the 75 had headache and pain in 21 patients (85.2%) of the 27 was completeley relieved or improved postoperatively. No significant differences of reduc-tion rate of rhinomanometric results between the headache and pain free groups were found. No significant differences of incidence of nasal allergy between the two groups were found. Neurotic ten-dency in the headache group was revealed but it was not statistically significant.
