　はじめに

　耳鼻咽喉科領域の患者が頭痛を訴えることにしばしば遭遇する。特に欧米では鼻副鼻腔疾患を有する症例にその頻度が高い^1）。急性副鼻腔炎における頭痛・顔面痛の責任部位や症状の程度との因果関係は比較的明瞭であるが，慢性の場合は単純ではない。慢性の鼻性頭痛については今のところ明確な定義もなく，またその病態についても曖昧である。しかし，本邦でのこの領域の研究は意外に少ない。そこで今回われわれは，慢性の鼻性頭痛・顔面痛の実体を明確にする一環として，慢性鼻炎および副鼻腔炎患者が有する頭痛・顔面痛について臨床的研究を行い若干の知見を得たので報告する。

In 150 patients (60 males and 90 females, mean age of 50 years) with chronic rhinitis and chronic sinusitis, we clinically investigated characteristics of headache and facial pain. Ninety patients (36 males and 54 females, mean age of 49 years) had chronic headache or facial pain. The characters of pain were dullness or dull headache (84.9%), sharp pain (11.0%), pulsative pain (2.7%) and compres-sive (1.4%). Majority of the severity of pain was moderate or between slight and moderate (66.6%). Head position (stooping), weather (cloudy) and excercise were concerned with relieving or exacer-bating the pain. Sites of pain were frontal (26.3%), temporal (18.1%), glabella (13.8%) and other por-tions. Relations between the portions of pain and X-ray shadow were not found. In comparison of the severity of pain with values of nasal resistance, rhinoscopic findings and grade of sinus X-ray shadow, no statistical relationships were found.
