　CT，MRIが副咽頭間隙腫瘍の診断において重要な位置を占めることに異論をはさむ余地はないが，CT，MRIには種々の撮影法があり，撮影法それぞれの診断的価値はまだ検討を要する。一般に用いられる撮影法として，CTならば単純CT，造影CT，MRIならばT1強調画像（以下T1），T2強調画像（以下T2），ガドリニウム造影（以下Gd）が挙げられる。これらの撮影法にはそれぞれ長所があり，すべて施行できれば当然，より正確な診断を期待できるかもしれないが，副咽頭間隙良性腫瘍例を診断する場合にも，はたしてこれらの撮影法すべてを施行する必要があるのだろうか。以前にも12例の副咽頭間隙腫瘍症例においてCTとMRIを比較検討したが^1），今回は単純CT，造影CT，T1，T2，Gdについてその有用性，必要性についての検討を，後述の読影指標の同定率の比較を中心に，30例の副咽頭間隙腫瘍についておこなった。

To diagnose parapharyngeal tumors by CT or MRI, several modalities for these imagings, i. e. plane and enhanced CT, T1, 2 weighted MRI and Gadolinium enhanced MRI, have been performed routinely. We evaluated the usefullness of these modalities in diagnosing parapharyngeal benign tumors in 30 cases. The usefullness of each modality was judged by identifying rate of four anatomical landmarks i. e. parapharyngeal fat tissue, the inter-nal carotid artery, styloid process and muscles attached to the styloid process. The combination of enhanced CT, Tl and Gadolinium enhanced MRI were useful to identify these landmarks, but T2 and plane CT had no effect for identifying the land-marks. CT is superior to MRI only in dipicting the styloid process, but if one method has to be chosen from CT or MRI, we concluded that MRI should be chosen.
