口腔咽頭に発症した尋常性天疱瘡症例

A Case of Pemphigus Vulgaris with Oral and Pharyngeal Manifestation

　はじめに

　尋常性天疱瘡は弛緩性水疱が全身に多発するのを特徴とし，ステロイド治療が導入される以前は難治でしばしば死に至る疾患であった。50歳前後の男女に多発する^1）。約半数の症例で口腔内に発症し，約5か月で全身に広がるため，病初期における診断と治療が重要で予後を左右する^2）。今回われわれは口腔，咽頭に限局して発症し，早期の診断によりステロイドの少量投与で寛解に至った1例を経験したので，若干の文献的考察を加えて報告する。

A 45-year-old woman complained of oral pain with bullous lesions in oral and pharyngeal mucosa. Bullous lesions developed to be pulurent ulcers.Laboratory data showed inflammatory reaction and HLA DR-6 which was known to be linked with pemphigus vulgaris. Biopsy was done, and HE study showed bullous formation in the superficial layer of basal layer. Clinical diagnosis was confirmed with indirect antibody titer 1 : 80, direct antibody to IgG and C3. Predonisolone 60 mg/day was administered and tapered to 20 mg/day in two months. The patient was discharged with indirect antibody rate 1 : 40 without any complication.
