副鼻腔手術後のtoxic shock syndromeの1症例

A Case of Toxic Shock Syndrome after Paranasal Sinus Surgery

　はじめに

　Toxic shock syndrome （以下，TSSと略）は1978年にToddら^1）によって初めて報告された症候群で，発疹，血圧低下，発熱などの主症状と下痢，筋肉痛，粘膜発赤，意識障害などの多彩な副症状を呈する疾患であり，重症例では致死的となることもある。欧米では鼻副鼻腔手術後に発症した報告も多く^2〜7），本邦においても副鼻腔手術後に発症した症例が数例報告されている^8〜10）。現在まで本症の確定診断に有力な手段がなく，発生頻度は潜在的に多いことが推察される。

　われわれはTSSと思われる1例を経験したので，その臨床経過とともに診断法について，文献的考察を加えて報告する。

Toxic shock syndrome (TSS) due to toxic shock syndrome toxin 1 (TSST-1) released by Sta-phylococcus aureus is sometimes fatal. Therefore,appropriate and urgent treatment of TSS are need-ed. Recently TSS cases following sinonasal surgery have been reported. However, the patients with the correct diagnosis of TSS seemed rare.

We experienced a patient with TSS after Cal-dwell-Luc operation, and the patient was treated successfully with hydration, appropriate antibiotics, γ-globlin, local irregation of disinfectant, etc, we discuss on making diagnosis and prediction of TSS.
