　はじめに

　頭頸部悪性腫瘍患者の手術で，腫瘍が頸動脈に浸潤している時には，頸動脈の結紮・切除を余儀なくされることがある。この場合脳血流を維持するため頸動脈の再建を行い，腫瘍を摘出することが多いが，われわれは脳血流の遮断時間をできるだけ短くするために，血管吻合する前にシャントチューブを適用することにしている。今回，頸動脈に癒着するリンパ節転移をもつ舌癌患者に対して，シャントチューブの適用に工夫をして手術を行ったので報告する。

A 75-year-old female with tongue carcinoma and metastatic cervical lymph nodes was operated with vascular reconstruction of the left carotid artery after glossectomy, total laryngectomy and left radical neck dissection. Before the neck dis-section and the vascular reconstruction, a tem-porary shunt technique between the right common carotid artery and the left internal carotid artery was applied. High flow arotic arch cannula (3.8 mm) connected with carotid artery shunt tube was used for the shunt operation. The left car-otid artery was replaced with the prosthetic graft, because of difficulty in the application of his own vein.
